Blood glucose monitoring plays an important role in
diabetes control. Tracking your results helps you spot trends and gives
your healthcare professional clues about your daily routine so your
management program can be adjusted. The more you know, the more you can be
in control of your diabetes.

For your convenience, the Logbook is available to
download and print.* The regular size of the logbook is similar to a
checkbook, so many of our customers like the larger 8.5 x 11 format when
printing from the Web.

Looking for an online Logbook?
Some customers would prefer to input their test
results into a software program on their computer instead of writing test
results on paper. Our OneTouch™ Diabetes Management Software features
reports, graphs and charts, and is available for FREE online.
Reorder a Logbook
Don't want to download? If you have a LifeScan
Logbook, you will find a reorder form in the back. The form allows LifeScan
customers to order a maximum of 2 Logbooks per year at no charge. Each
Logbook holds seven to eight months of blood glucose results and insulin/medicaton
doses.
To order additional Logbooks, please complete the reorder form located in
your Logbook and mail the pre-addressed card. If the form is missing, you
may request Logbooks from LifeScan Customer Service Department, 800
277-8862, or e-mail your request (name, mailing address, phone number and
meter serial number). Please allow 6-8 weeks for delivery. Reorder applies
to U.S. customers only.
* Same
day and next day shipments are normally the case with the exception of any
out-of-stock items.
