IMPORTANT! READ ALL DIRECTIONS BEFORE USE

The MyAllergyTest is used with a fingerstick blood sample to determine if you have allergies to 10 of the most common allergens. The MyAllergyTest, a home allergy test, is for the collection and transport of a capillary blood sample to a clinical laboratory for analysis and reporting directly to the consumer allergen specific IgE test results. No physician order is required for a consumer to purchase MyAllergyTest, order that the test be run, or receive test results. MyAllergyTest is intended for use with the MyAllergyTest System used in the clinical laboratory that runs the test and reports test results to consumers. MyAllergyTest is intended for home use by the lay consumer (Over-The- Counter). 

The 10 allergens tested are:

Timothy Grass Egg White
Bermuda Grass Milk
Mountain Cedar Wheat
Short Ragweed Cat
Alternaria (mold) House dust Mite

For non-food allergens, the MyAllergyTest System correctly identified 93 percent of the cases, and for food allergens, the MyAllergyTest System correctly identified 90 percent of the cases, when the results were compared to results obtained with another, standard laboratory testing system.1

NOTE: High fat meals may affect test results. For best results, collect the blood sample before eating or several hours after a meal.

STEP 1: GET PREPARED: Get to know your MyAllergyTest kit

The home allergy test kit includes a Pouch containing:

Read the Directions
This is very important: Before you stick your finger, read all directions carefully.

Also read the section "What can I do to make the blood collection easier?" on the back of this insert.

Prepare Your Cards
Fill in all the required spaces of the Registration Card. Please print clearly.

The MyAllergyTest Laboratory must receive your Registration Card and your blood sample within 10 days of blood collection. Please mail your sample promptly

Write in the date of blood collection on the Personal Information Card, attach your PEN, and keep for your records.

Wash and Dry Hands
Wash your hands with soap and warm tap water.

Dry your hands thoroughly.

Assemble Blood Collection Tube
Push the funnel through the cap of the blood collection tube, making sure the wing of the funnel points to the corner of the cap. (The cap has an "X" cut in it.)

STEP 2: GET A BLOOD SAMPLE

High fat meals may affect results. For best results, collect the blood sample before eating or several hours after a meal.

Choose Finger and Puncture Site
Choose your middle or ring finger. Your finger should be warm and callus-free.

For best results, choose an area to the side of the soft pad of the fingertip, on the side of the finger closest to your little finger.

Use the alcohol pad to clean the fingertip.

Save the alcohol pad for use after taking your blood sample.

LET YOUR FINGER DRY COMPLETELY.

Shake Your Hand
Stand up and let your hand hang down. Shake your hand to increase the flow of blood.

Stick Your Finger
Lay your hand on a clean flat surface to support the finger you are going to stick,

Hold the Safety Lancet between the first and second fingers of your free hand, as seen in the picture.

Push the Safety Lancet firmly against the chosen fingertip.

With your thumb, press the white button on the Safety Lancet until you hear a click. This will take some force.

Throughout blood collection, keep your hand below your heart. Standing up should help the flow of blood.

You may not see any blood until you squeeze your finger.

From a standing position, gently "milk" the hand and finger toward the fingerstick site. Wait a few seconds between squeezes to allow the finger to fill with blood. When the fingertip is filled with blood, gently squeeze the finger right above the fingerstick site. Do not squeeze the finger too hard. Doing so may affect the blood sample.

Always keep your hand below your heart.

When you see a drop of blood, let the blood flow into the funnel of the Blood Collection Tube. If necessary, you may scrape the blood into the funnel.

Caution: Blood begins to clot in about 9 seconds. Please collect your sample as quickly as possible.

Graphics (p.3-10) Continue to gently "milk" the hand and finger toward the fingerstick site, and let the blood flow down into the funnel of the Blood Collection Tube.

Keep collecting blood until you have collected 4 or 5 drops.

If necessary, use the second lancet, and repeat the above process.

Graphics (p.3-12) Holding the wing of the funnel, tap the Blood Collection Tube on a table until the blood flows out of the funnel and into the bottom of the Tube.

Apply Bandage
After collecting your blood, wipe your fingertip with the alcohol pad, and apply the bandage to your finger.

Remove the Funnel
After tapping all of the blood down into the Tube, pull the funnel out of the Blood Collection Tube.

Make sure the Blood Collection Tube cap is on securely.

Safely dispose of the used Safety Lancet, collection funnel, and all other used materials in your home garbage.

STEP 3: ATTACH THE BAR CODE LABELS

Attach Label 1 to the Personal Information Card. Keep this card in a safe location. You will need your PIN (Personal Identification Number) if you choose to get your test results from the Internet.

Attach Label 2 to the Registration Card.

Graphics (p.4-6)

Write your name on Label 3 and attach it to the Blood Collection Tube, so that the bar code and number ends are back to back, as seen in the picture.

Attach Label 4 anywhere on the back of the pre-addressed Foil Shipping Envelope.

STEP 4: MAIL YOUR BLOOD SAMPLE

  1. Check your Blood Collection Tube. Make sure it is fully closed and that Label 3 is attached to it.
  2. Leave the small paper towel in the Bubble-wrap Envelope.
  3. Put your Blood Collection Tube into the Bubble-wrap Envelope, and seal well.
  4. For extra padding, wrap the Bubble-wrap Envelope around the Blood Collection Tube.
  5. Put the Registration Card and the Bubble-wrap Envelope in the Foil Shipping Envelope, and seal well.
  6. Place in any US mailbox as soon as possible.
  7. The MyAllergyTest Laboratory must receive your Registration Card and blood sample within 10 days of blood collection.

STEP 5: GET YOUR RESULTS

Once you have mailed in your blood sample, please allow 10 days for the sample to arrive at and be tested by our laboratory.

There are two ways to get MyAllergyTest results:

  1. On the Internet. If you provide an email address, when your results are ready, you will receive an email directing you to our website, www.ImmuneTech.com.

   2.   In the U.S. mail. If no email address is provided, results will be mailed to the address on the Registration Card.
