| Locate &Collect the inexplicable body fluid
stain |

Using a UV Light you can illuminate
inexplicable body fluid stains. The stain will "glow" a purple
color, works best in a darken room, such as a closet.

Collect the inexplicable body fluid stain using the
CheckMate Semen Detection Kit. Which includes 5
tests. You may also purchase the CheckMate Double Kit
which includes 10 tests. |

This is NOT a saliva test--cheek cells are needed for
testing. |

Open the plastic swab package. Remove the swab from
the package. Scrape & rotate the swab inside of the cheek 10 times
(about 15 seconds).

After collecting the cheek cells using the swab, place
the swab directly into the appropriate envelope. Label the envelope
with your information. |

Place the CheckMate Collection pad in another envelope with
correct information labeled on outside. You may also cut around the
inexplicable body fluid stain and place the piece of fabric in the envelope. |
| Packaging the Samples |

Keep the swabs in the appropriately colored (and labeled)
envelope, and seal it shut. NOTE: These samples are
stable at room temperature for several months. Repeat from step 3
for each of the other individuals being tested. |

Enclose any other specimens, such as hair or biological
stained garments, in a plastic or paper container, and label the container
appropriately. NOTE: All biological stains must be dried
before enclosing them into plastic containers. |
| 9. |
Make sure that all the samples are appropriately and
clearly labeled. Place all the envelopes and specimen collection bags
containing the specimens into a cardboard mailer, and include all the
completed forms. Once all the specimens and forms have been placed in the
cardboard mailer, mail it (via US Mail, UPS, FedEx, or Airborne), with
your payment. |
