Three Steps to Determining Paternity for Legal Purposes:
| 1) | Submit the 'Order Form'
to request the initiation of your specimen
collection appointment scheduling.
You have the choice of using the BloodDraw™ or GeneSwab™ method of specimen collection. Both are recognized in most counties for obtaining suitable specimens used in determining paternity for legal purposes. However, you may want to contact an attorney in the county you reside in to assure yourself that you are selecting a suitable method. We require a $200 deposit prior to having the appointments scheduled. All appointments will be scheduled within a 30-mile radius from your home or business. There is an additional $25 fee for each person being collected outside San Jose, CA, or Salt Lake City, UT, unless other arrangements are made. If you are not satisfied with the location or appointment times we provide you with, your deposit can be refunded. After the appointments have been made, you will receive the schedule either by fax, mail, telephone, or email, which ever you prefer, within 48 hours. The alleged father and child's specimens are required to determine paternity, but we ask that if the mother is available for collection that she submits her specimens for analysis as well. Additionally, if the participants that are being collected are located in different regions we can set up collection appointments at multiple locations across the country. There is an additional $25 fee for separate collection sites. NOTE: if prenatal testing is requested, the name and phone number of the doctor performing the Amniocentesis or CVS is required at least 4 days prior to the scheduled procedure. GeneTree must send the instructions and appropriate materials to the doctor at least 24 hours before the procedure. |
| 2) | Fingerprints and Polaroid pictures will be taken at the
time of collection for proper identification of subjects
in the study.

| 3) | After collection the specimens will then be sent for analysis. Results take about two weeks, and will be provided either by email, telephone, mail or facsimile, whichever you prefer. A notarized copy will also be sent, at your request, to the court where the proceedings are being held. |
