This specimen collection bag has a large pouch for chain-of-custody forms, a smaller pouch for test specimen, an absorbent cloth in smaller pouch, and self-adhesive flap to seal both pouches.

Instructions

1. Place Chain of Custody form in a large pouch.

2. Place specimen in smaller pouch.

3. Remove release liner from flap.

4. Fold blue adhesive flap to cover black cross hatch slit opening.
