PSA4 Instructions 

Precautions and Limitations

For in Vitro diagnostic use only. Do not take internally.

Failure to follow all instructions may affect the accuracy of the test result and may result in an inability to analyze the blood sample and report results.

Use only with finger nick (capillary) blood from humans. Not for use with any other body fluids or food.

Store this kit at room temperature (below 86 degrees F/30 degrees C). Avoid freezing. Once the blood sample has been collected it is perishable. The blood sample must be received in the provided "Blood Sample Return Bag" within three weeks or it may not be tested.

Once you have collected your blood sample do not store the sample at or above 50 degrees C or 122 degrees F for more than 12 hours.

WARNINGS!

Not intended for use by hemophiliacs or those persons taking medications that thin the blood (anticoagulants) unless otherwise directed by your Health Care Professional.

Never adjust your medication based on the test kit results. Always consult your Health Care Professional first.

Use the provided lancets on only one individual per test kit. Never use a lancet that has been used by another person. Keep out of reach of children. This kit is not for neonatal testing.

INTENDED  USE

This kit is intended for the Collection of a Capillary Blood Sample from a finger nick. For in vitro diagnostic use only.

Carefully remove all parts of the kit and place on a clean dry surface.

Check that all parts are in the kit. (See Kit Contents). (Figure 1). If any part is open or broken, do not use.

Graphics (p.4-3)2. INFORMATION REQUIRED
Make sure that the Laboratory Authorization Form has been properly filled out and that all areas on the form are complete and accurate. (Figure 2). 

Please note the Date and Time of blood collection on the Authorization Form, where indicated.

Remove the Blood Collection Card from bag marked "Blood Sample Return Bag". Print your name on the Blood Collection Card.

 

Graphics (p.5-1)3. SAMPLING PREPARATION
Wash hands with warm water and dry thoroughly.

Choose either the "ring" or "middle" finger for the test. It should be clean and warm. Choosing an area slightly off center produces a better blood flow. 

To increase blood flow, rub hands together for 10 seconds and then allow arm with selected finger to rest at your side for 30 seconds. Standing may also help blood flow. 

Use the enclosed alcohol prep to further clean finger selected for sampling. Let, alcohol dry completely from finger before proceeding. 

Place the chosen hand with palm side up on flat surface such as a table.

4. DRAWING A BLOOD SAMPLE
Use enclosed finger lancet to get a blood sample. (Figure 3).

Remove lancet from package. Place tip at side of your finger, hold steady, then press down firmly on the raised button, then release. 

It is recommended that you do not use the center of your finger, since this is the most sensitive area of the finger. 

Observe the finger to determine if blood is flowing from the nick. It may be necessary to squeeze the finger from the base up to the nick site to produce the first drop of blood.

With sterile gauze pad wipe away the first drop of blood that forms.

Graphics (p.6-1) 5. BLOOD SPOT COLLECTION

It is acceptable for blood to extend outside the circle.

Hold finger down over the first circle on Blood Collection Card but do not touch the circle.

Graphics (p.6-2)When a full hanging drop of blood is formed, gently touch the blood drop inside the first circle. If needed, continue to apply blood drops onto open areas of the first circle until it is completely covered. Do not layer blood drops on top of one another. (Figure 4).

FILL THE FIRST CIRCLE COMPLETELY BEFORE GOING ONTO THE SECOND CIRCLE.  Fill the second circle completely, as above, before going on to the third circle. Completely fill all circles on the Blood Collection Card. FAILURE TO FILL ALL CIRCLES COMPLETELY WILL RESULT IN A SAMPLE INSUFFICIENT FOR TESTING (Figure 5).

After filling all of the circles completely, apply the adhesive bandage to finger. 

Graphics (p.7-1) Allow Blood Collection Card to air dry in a clean, dry area for 10 minutes. Do not allow Blood Collection Card to be exposed to direct sunlight or extreme temperature or humidity.

6. MAILING YOUR BLOOD SAMPLE
Promptly after drying the Blood Collection Card for 10 minutes, insert it into the bag marked "Blood Sample Return Bag" and seal. The desiccant should remain inside the bag. (Figure 6). 

Graphics (p.7-2) Place completed Laboratory Authorization Form and sealed "Blood Sample Return Bag" into the preaddressed, prepaid, "Business Reply Envelope" (Figure 7).

Place in regular mail as soon as possible. (Figure 8).

 

Graphics (p.8-1) YOUR TEST RESULTS
BioSafe Laboratories will process your test once the blood sample is received. You and/or your Health Care Professional will receive test results within 10 days of receipt of blood sample.

Be sure to discuss the results of this test with your Health Care Professional. In addition, this BioSafe Laboratories result should not be compared with other results done by a different method. Different laboratories may test differently and give different results.

Graphics (p.8-2) CLEAN-UP PROCEDURES
Dispose of lancets properly.

Do not reuse the lancets. They are designed to be used only once.

Dispose of the used lancets in the disposal container provided with this kit. Screw lid closed and discard this container and all other used materials with your daily household trash. (Figure 9).
